A second investigation is believed to be underway into the death of Margaret Seery (42).
The Irish Independent reports the Limerick woman was buried without her major organs in 1994.
Her death was not treated suspicious at the time, but fresh information led to her remains being exhumed 10 days ago.
At the time, it was believed she had choked to death. But it is understood a man contacted gardaí with new details in the case.
It was reportedly discovered her organs were missing during a post-mortem as part of that investigation.
The paper reports there are no records of her organs being donated, and another inquiry is now underway to determine what happened.
