A couple who lost two babies have been told they have to give the details of their ordeal all over again.
It comes as a maternity review into births at Portiuncula Hospital in Co Galway has reportedly been further delayed.
Two new investigators have been assigned to the process.
The review started back in March of 2015, and was due to be completed within five months.
Warren and Lorraine Reilly's baby, Amber, died there in 2010. At her inquest, the coroner requested the review - which is on-going.
The couple also lost baby Asha at the same hospital in 2008.
Mr Reilly told Newstalk Lunchtime he and his wife will have to re-tell their story all over again.
"Initially they told us that their review team would be able to do the rest of the chronology based on the notes that the investigators had taken".
"But in the last few days they have informed us that they are going to assign new investigators, and we will have to go through the investigation and interview process all over again".
