The devastation caused by an explosion at a hospital in Gaza has been met with widespread condemnation.  

However it is still unclear who is responsible for it – both sides are laying the blame at the other’s feet.  

EU Commission President Ursula von der Leyen said “All facts need to be established, and those responsible must be held accountable.” But is that even possible?
