A report into the death of baby Mark Molloy at Portlaoise Hospital in January 2012 has found two main care delivery issues in his death.
The HSE says the staff failed to recognise and act on the signs of foetal distress.
The report is published today at the request of the Molloy family, it makes 43 separate recommendations.
The HSE has again apologised to Mark and Roisín Molloy for the events that occurred on the 24th of January related to Roisín's labour and delivery, and for the death of baby Mark.
Mark Jr died 22 minutes after he was born.
Mark's father told Newstalk.com: "There is always going to be a cloud over our house. Certain days - the first days of school, or when a text comes in about a match will be even harder... But if there can be learning derived from Mark's report, and maybe other little boys and girls get to go home, I suppose we can take some comfort in our effort - four years has not been in vain."
