This morning the Health Information and Quality Authority (HIQA) published its long-awaited report on child deaths in the Midland Regional Hospital in Portlaoise.

In a shocking indictment, the report found significant deficiencies and concerns over services at the hospital, and that the HSE failed to take action at a national level. It also said it cannot guarantee services are safe at the moment.

Warren Reilly’s daughters Amber and Asha died in Portiuncla hospital in Ballinasloe and serious failings in the management of her mother Lorraine’s labour were found there too .

Some of these issues have resurfaced in subsequent incidents at the hospital which are to be the subject of a forthcoming inquiry.

Today, Warren spoke to Jonathan Healy on Lunchtime about the report and stated about the report: "It's made me so resolute that something has to be done for the families involved in Portlaoise, for the families involved in Portiuncla, for the families involved in Ennis, Mallow and other hospitals and also for thousands of families to come, that's what this needs to be about.

"It's not just about those of us who have had sad losses in the past but to avoid that happening needlessly in the future, it just has to be changed."
