A number of healthcare staff involved in the care of Savita Halappanavar have been "disciplined".

Savita died at University Hospital Galway in October 2012, a week after she was admitted to the hospital when she was 17 weeks' pregnant and miscarrying.

Enquiries into the death of Savita Halappanavar identified a number of shortcomings in her care at University Hospital Galway. The group responsible for the hospital started a review process to examine all staff members involved.

Of the 30 staff whose actions were considered, 21 were found to have no case to answer and weren't disciplined.

However, some of the remaining nine members of staff received written warnings which outlined the necessary improvements they need to make and a timescale in which to do it.

Others were told to take pre-procedural counselling which would inform staff members what aspects of their work needed to improve.

The West Northwest Hospital Group said the disciplinary procedures against these staff members is now complete.
