The Garda Inspectorate has set out more than 200 recommendations for streamlining the investigation and management of crime.
The report, which is due to be released on Tuesday, also suggests reform of management structures in the Gardai.
The Sunday Times says the Inspectorate examined more than 150 incidents over 3 years and found that some crimes were not recorded on the computerised Pulse system.
In other cases, the investigation found evidence to suggest that gardai didn't respond to certain types of crime, including reports of domestic violence.
Draft copies of the report have already been presented to the Minister for Justice Frances Fitzgerald and the acting Garda Commissioner Noírín O'Sullivan.
