Cork's Mercy University Hospital has been told to carry out essential maintenance works on the Intensive Care Unit (ICU) as a matter of priority.
It follows a surprise inspection by the Health Information and Quality Authority (HIQA) in July.
Inspectors found the ICU infrastructure and design was dated and did not meet the desirable standards of a modern day critical care facility, nor did it facilitate effective infection prevention and control.
It was found not to have appropriate isolation facilities for patients with transmissible infection.
HIQA inspectors say there have been improvements in cleanliness and hygiene in wards since an inspection in October 2014.
However, there were still some issues regarding equipment cleaning and dust was present on a number of surfaces.
The 319 bed facility is shortly to become the Regional Centre for Gastroenterology and a recognised site for the National Colon Cancer Screening Service.
