It's emerged that a HSE emergency ambulance - sent to help the family of a child who suffered suspected cot death - was sent to the wrong county.
The incident happened when the 3-and-a-half-week-old child stopped breathing in Co. Kerry earlier this week.
The 999 emergency call was made around 1am on Monday morning.
The family of the infant were living at Tennis Village in Tralee - but the initial ambulance was sent to the Tennis Village on Cork's Model Farm Road.
The first responders attended the correct address some 30 minutes after the call.
Since last month, all 999 calls have been answered by dispatchers at a new ambulance control centre in Dublin.
In this case, the HSE say they are satisfied that "necessary protocols" were adhered to.
This latest incident will do little to reassure the public about the reliability of the ambulance service, despite HSE assurances that improvements are being made.
