The husband of a woman who died during surgery for an ectopic pregnancy says he doubts some aspects of a report into the his wife's death.
Last month, a coroner returned a verdict of medical misadventure in the case of 35-year-old Malak Kuzbary Thawley.
She died during surgery for an ectopic pregnancy in May last year.
Malak, a teacher from Syria, was advised to undergo the surgery after scans confirmed she had an ectopic pregnancy.
She died during the operation.
Ms Thawley and her American husband, Alan, had been living in Ireland for a number of years.
He is now calling for an independent inquiry into her death.
Alan Thawley told Pat Kenny about when they went to the National Maternity Hospital, after discovering it was an ectopic pregnancy.
"They did another scan and they found it, and so we had made the decision to go into surgery - which was the right decision for that situation.
"There was really no emergency, I think we even waited over an hour before the surgery began because they wanted more time between when she ate breakfast to actually giving her the general anaesthetic.
"It was a very casual way to start the surgery, there was no emergency really".
"We were give a lot of assurances... I remember arguing for methotrexate - which is a drug that can be given that can abort the ectopic pregnancy, but they told us the standard procedure is surgery.
"He even assured me saying 'listen we do this 30 to 40 times a week, it's a standard procedure, she'll be in and out in less than half an hour'".
"She was already bleeding to death"
Alan says after an hour, it became clear something was wrong: "It was a little more than an hour after the surgery began and I still hadn't received an update and so that's when I was beginning to get worried.
"I did have some people who tried to come and give me an update, but the updates were extremely vague - I could tell they actually had no idea what was going on but they just wanted to talk to me".
Alan says the inquest revealed what went on in the operating theatre: "The first instrument that's used to make the first incision, that instrument actually cut my wife's aorta.
"So really what that means is within the first seconds of the surgery she was already bleeding to death".
Alan says he now wants answers: "I've been alluding to this report that the hospital created and part of the problem is there's lot of information tat was missing.
"One of the very big issues I have is that this hospital killed my wife through their multiple and serious failings - and they are the same people who have to give me all the evidence as to how my wife died."
He also says the Master of the National Maternity Hospital, Dr Rhona Mahony, has been "silent since day one in public".
"To me it feels like she's actually ignoring the issue, she's actually making it feel like I'm not important and my wife's death is not important".
In a further statement, the National Maternity Hospital said: "The Master of the National Maternity Hospital Rhona Mahony met Mr Thawley both on the night of the tragic death of his wife Malak and the following day.
"She would very much like to have continued meeting with him, answering all the questions he had and providing some assistance as he began the grieving process following the devastating loss of wife while she was in our care.
"However we were instructed then by Mr Thawley’s solicitor to have no further contact with him.
"We abided by this instruction, though felt then and still feel that ongoing contact with hospital staff would have allowed Mr Thawley get a full understanding of what had happened.
"We believe the hospital report and the transcript of the inquest do answer many of the questions Mr Thawley clearly still has and clarifies the cause of death, which was a vascular accident for which the hospital has always admitted full liability.
"The entire staff including Dr Mahony are deeply traumatised by this event.
"Our apology is heartfelt and repeated and our thoughts are with Mr Thawley. We understand that no words can bring Mrs Thawley back."
