An internal review is due to take place into the death of a woman who gave birth at St Luke's Hospital in Kilkenny this week.
Tracey Fitzpatrick (36) died after giving birth to a boy on Easter Monday.
She was married in 2013, and this was her third child.
She was taken for surgery at the hospital, but it is thought she had already passed away.
Southern correspondent with the Irish Independent, Ralph Riegel, explains what happened.
"Ms Fitzpatrick was brought to the hospital in Kilkenny, she gave birth to a healthy baby boy on Easter Monday - but within minutes of delivering a baby which was called Max her condition began to deteriorate" he told Newstalk Breakfast.
"Hospital staff were very concerned for her because of her various vital signs in monitoring".
"She was immediately taken for emergency treatment to an operating theatre, but sadly she died very shortly after being brought there" he added.
