The violence at the 400-bed Sardar Mohammad Daud Khan hospital began when a suicide bomber blew himself up at the rear of the building, Reuters reports.
Gunmen dressed as doctors are reported to have fought security forces inside the building for several hours.
More than 60 people who were wounded have been taken to other hospitals.
Afghan President Ashraf Ghani said the attack “trampled on all human values".
The attack was condemned by Jens Stoltenberg, the Secretary General of NATO:
I strongly condemn the inhumane #Kabulattack on hospital patients and staff. #NATO stands with #Afghanistan in the fight against terrorism.
